Noble Hospice Care allows patients to choose whether they would like their Primary Care Physician or our Hospice Medical Director to navigate their hospice care plan. In partnership, your PCP and Hospice Medical Director will develop a prognosis for receiving hospice care.​

Noble Hospice provides a trained hospice RN Case Manager as needed to visit patients. Typically, they will visit the patient a minimum of twice a week to assess and manage care. Noble Hospice also has 24/7 access to on-call RN’s to triage and dispatch when there are acute issues.​

Hospice Aides are local heroes that make a positive impact on the lives of the sick, disabled, and elderly. They assist patients with completing everyday personal tasks such as dressing, bathing and a variety of hygiene needs. It includes assistance in everyday duties such as shopping, transportation, and medical appointments​.

Medical social workers create long-range plans, such as advance care planning and educate both patients and their families about community resources and support.

Volunteers are available to help enlighten our patients and to spend companion time. Volunteers often times will allow a small respite break for the family or caregiver while in the home. Noble Hospice and Palliative Care always looks for the bets volunteers that will be able to interact with our patient population or be able to provide support from a administrative approach.

Spiritual Care Coordinators (Chaplains) are provided to assist with making patients comfortable and at peace, while providing family members with a pillar of support during this difficult time.​

Marketing Liaisons are the first point of contact when a patient is referred to hospice or palliative services. They help educate the community on the benefits our services have for our patients and support the clinical team in any way necessary. 

Administrative staff includes our admissions team, HR, administrative assistant, office manager, and digital marketing. They support the organization from behind the scenes.
